Community Residential Settings
Corporate Adult Foster Care
Our residential programs provide direct services and supervision 24 hours a day, 365 days a year. In these settings, supports are designed around the needs of each individual with emphasis on health, safety, and independence.
Programming includes:
- Instruction and supports in completing Activities of Daily Living (ADLs)
- Medication Administration
- Money management monitoring
- Health management and coordination of all health service appointments
- Assistance and supervision for community involvement
- Meal preparation and coordination
- Personalized programming to help individuals reach their goals
It is our primary objective to provide person-centered planning and service delivery. We believe in putting the person at the center of care.
Currently we serve individuals through 5 adult foster care homes in St. Peter, Le Center, and Montgomery, MN, and one Housing with Services customized living location in Montgomery, MN.
